How does a POS plan work?

A POS (which stands for Point Of Service) plan mixes some of the features of HMO and PPO plans. Similar to an HMO, POS plan members have to choose a PCP from the plan's list of providers. Services provided by your PCP are typically subject to a deductible. Like HMOs, POS plans typically offer coverage for preventive care examinations and doctor visits.
However, typically you will only get a higher level of coverage for services provided or referred by your PCP. Services provided by an out-of-network provider may be held to a deductible and may be covered at a lower level. If services are provided by someone outside the network, you may have to pay upfront and submit a claim to the insurance company personally.
